Why do some ice cubes look crystal clear, and others look sort of foggy?

It depends on the purity of the water and on the amount of air suspended in the water. If the water is very hard, or has a lot of air suspended in it you will get cloudy ice cubes. That's why for ice sculptures they like to use destilled water and they leave it sit for quite a while after filling the container to let the air rise out.
